Abstract: An unfortunate railway accident had happened on July 23, 2011 inChina. Also, many safety & quality accidents of products had happened in recent years. So, how to prevent the quality accidents before they happen is discussed in this paper. By the theory of Process Method in quality management, there are three compositions of input, processing and output in a process. To obtain the expected result, preventive actions should be taken in all Input, Processing and Output phase. A new PACC (Prevention, Action, Check, Cleaning) cycle is proposed in Processing to take preventive action. Input is controlled from4M1E (Man, Machine, Material, Method and Environment) and output is extended to customer to prevent product being used unexpectedly. Also, a new meaning had been ascribed to4M1E in management besides its’ original meaning. How to run PACC cycle based on PDCA is introduced then. Last, the limitation and the future research are mentioned.
